Please Note: If your most recent League membership was with another League, please contact your former League first. Your membership must be reinstated through your past League, and they can then initiate the transfer process for you. By submitting this form, I acknowledge and understand that:

If I voluntarily resigned or otherwise chose to end my Active membership with the JLDOC, the Management Team will vote on my reinstatement at their next monthly meeting.

If I was for any reason dropped from Active membership with the JLDOC (for example, for non-payment of dues or otherwise failing to meet requirements), the Board will vote on my reinstatement at their next monthly meeting.

If I voluntarily or involuntarily ended my membership as a Sustainer, League leadership will communicate with me re: the formal reinstatement process (Board or Management Team vote at next monthly meeting, depending on circumstances of termination).
