Community Assistance Fund Grant Report

This form is for organizations that have been awarded Community Assistance Funds from the Junior League of Odessa.

If funded, organizations are required to submit a Post-Project Report within 45 days of using the funds. The report must include:

- A financial summary with supporting receipts and/or invoices
- A description of the project’s outcomes, including whether the stated goals were achieved
